Turning Compassion into Measurable Health, Workforces and Community Outcomes
KCC is a 501(C)(3) nonprofit dedicated to improving health and well-being by addressing Social Drivers of Health (SDOH). We believe that true health goes beyond medical care it is shaped by the connections, environments, and support systems around us.
Through our Compassionate Care Networks, we create community-based health and Wellbeing plans that empower individuals and organizations to build healthier, more resilient communities.
Our work is grounded in three key pillars:
Workforce Well-being: Supporting professionals and human beings in high- stress industries and organizations to prevent burnout and foster well-being on both and organizational and personal level.
Whole Care Health – Humanizing care by addressing patients' social needs and strengthening provider resilience.
Compassionate Neighborhoods – Engaging communities to find their own solutions, amplify their voices, and build lasting support networks.
By listening, collaborating, and innovating, we help people connect, heal, and thrive together.
Workforce - Well-being
Evidence based resilience, purpose and compassion programs for employees and communities.
Up to 40% of employees face burnout across industries. This is not simply a wellness issue; It is a business risk affecting retention, performance, culture, and cost. We offer a hybrid model combining evidence-based mindfulness and compassion training with AI- enabled analytics dashboards that identify well-being, risks and track engagement.
Burnout, affects professionals across industries, healthcare workers, educators, social workers, first responders, and many others who dedicate themselves to serving others. The emotional weight of caring for people, engaging with their struggles, and responding to high-stress situations can lead to progressive disconnection from life’s purpose, psychological strain, and emotional exhaustion.
We develop humanization strategies that address burnout, improve workplace well-being, and enhance the quality of care and service. Our Compassionate Mindfulness Training Program equips professionals with tools to reconnect with their purpose, build resilience, and foster deeper human connections—helping them care for others while also caring for themselves.
Whole Care Health
Integrating Social Care into clinical workflows to reduce utilization and costs.
Social Drivers of health and loneliness have a higher impact on health outcomes than many health interventions. Healthcare organizations need practical models to identify and address these drivers at scale. We offer a Community Health Worker (CHW) model that integrates social care into clinical workflows and supports better health outcomes and more coordinated care.
We recognize the profound impact that loneliness and social needs have on health outcomes. That’s why we integrate assessments of social connection into healthcare, ensuring a more holistic approach to care.
Our strategies help the healthcare sector go beyond treatment connecting with the true needs of patients, providing meaningful support, and fostering stronger communities. By addressing these social factors, we improve overall well-being and create a healthcare system that truly cares for people and their communities.
Community activation model to reduce loneliness, improve well-being, and social capital.
The need for innovative solutions to address Social Drivers of Health (SDOH) is more urgent than ever. At KCC, we believe that the most effective solutions come from within communities themselves.
Our bottom-up approach prioritizes community voices and lived experiences. By actively listening to residents, we support them in identifying resources, developing their own strategies, and driving meaningful change. Through these efforts, we help lower negative health outcomes linked to SDOH and connect communities including COMPASSIONATE NEIGHBORHOODS with key stakeholders who can amplify their impact.
By empowering people to shape their own future, we create stronger, healthier, and more resilient communities.
We are thrilled to spotlight our Executive Director, Camila Ronderos Bernal, in her recent feature with Social Prescribing USA. In this interview, Camila shares her insights and vision for the future of social prescribing in the United States. She highlights the heart behind Keralty Compassionate Communities, reminding us that sometimes the most powerful health "intervention" is simply a community that cares.
